Baptism Registration Form

Rev. 7/30/2021

Requirements for Baptism of Children

1. At least one parent must be a member of the parish (i.e. living within the parish boundaries or formally registered). If neither parent lives within our parish boundaries nor is registered at the time baptism is requested, the parent will be referred to their respective parish. Non-parishioners may baptize their children in this parish but they must first submit a letter of permission from the pastor of their parish.

2. The parent(s) or legal guardian(s) must be a practicing Catholic. In addition, the parent(s) or legal guardian must attend Mass regularly.

3. Godparents must be practicing Catholics in good standing in their respective parishes. One godparent is sufficient; however if two are chosen, one must be male & the other female. Godparents must provide proof of having received the Sacraments of Initiation: Baptism, Communion, and Confirmation and if Married must be married in the Catholic Church and be practicing the Catholic faith. Anyone cohabitating (living together) may not serve as a Godparent.

4. Parents and godparents are required to attend a three-hour Baptism preparation class that will be held the second Saturday of the month. Failure to attend the class will result in the postponement of the child’s baptism. Only those persons who have completed their paperwork by the deadline are allowed to attend the class. No children will be allowed into the Baptismal class since this can be a distraction to those participating in the class. Anyone arriving late to class will be asked to return at a later date to complete the required class. This class may be taken at another parish; however, proof of this fact must be provided to our office by the day of the class. (Schedule & location of class are subject to change.)

5. Baptisms will be celebrated on the last Saturday of each month, except for the month of December, when there will be no baptisms, and during the Lenten and Easter seasons, when Baptisms will be celebrated on Easter Sunday. All paperwork, including the completion of the Baptism Registration Form, is to be completed and turned in before or by the first Friday of each month in order to be eligible to attend classes that month and to have your child baptized that month.

6. The donation for the preparation for the celebration of Baptism is $30.00 per child. (The baptismal candle will be provided by the parish; however, parents may choose to purchase their own.) All children are asked to wear white garments on the day of their baptism. Baptism Classes: A donation of $10.00 per person is requested of anyone who wishes to baptize at another church but would like to attend Baptismal classes at this church.

Note: All those who have participated in Baptism Preparation Classes will be issued a certificate, which will be valid for three years. Failure to obtain this document or failure to produce this document for future baptisms will require the individuals to repeat the class. It is the sole responsibility of each participant to save this document for future use
